TEAMS ARE REQUIRED TO BRING TWO COPIES OF THE FOLLOWING (COMPLETED) MID-CITIES FORMS TO THE TOURNAMENT

  • Mid-Cities Media/Medical Release for Team Members
  • Mid-Cities General Release (over 18) for Team Managers
